The Department of Internal Operations goes after Mary Sues that have infiltrated HQ itself. That it even exists is not common knowledge - in theory, only the Flowers and members of the department itself know about it - and all non-members are neuralysed on contact with it. It is headed by the Elm.
According to Peter Piper, if the DIO's existence were to become common knowledge it would cause a public outcry against the Board of Department Heads that would most likely result in a second Reorganisation and the deaths of the DIO's members (DIO mission 1: "Nita, you were chosen in part because we knew that your reaction to this would not be too severe. Most agents are not like you. Especially among the older members, there's too much of the DIS about us. We would be dead if they knew about us, defences or no.").
The DIO was created as one of three new Security Departments (along with the DIA and the DES) in the wake of the Reorganisation and the destruction of the DIS. It is located inside a maze of urple, wilver, bleen, and blello walls, laced with gravity distortions which all DIO Agents must learn to navigate blind. Even if this defence is somehow penetrated, Agent Justin Agent is waiting with his axe to stop any infiltration. The DIO also operates the Uber-Neuralyzer for decommissionings; the decommissioned agent's partner is allowed to attend, but must be neuralysed afterwards.
Besides Justin, who runs the Control Centre, the only known DIO Agents are Peter Piper, Nita Kerys and Lazarus.
